Program for High Hazard class codes WITH good loss experience:
Criteria:
- $5,000 minimum premium
- $5.00 minimum rate
- 3 years with continuous WC coverage
Send us a fully completed Acord 130 application, loss runs and Experience modification worksheet. Also need 5 years premiums and total payroll by policy period.

Program for High Hazard WITH High Experience Modification and/or Poor Loss Experience:
Target Risks (flexible):
- $20,000 minimum premium
- 9.00 minimum rate
- $24,000 minimum payroll per employee (including part timers and covered officers)
- 6 W-2 employees minimum
- Non-Renewed or cancelled standard market risks, Assigned Risks, High ARAP
Do NOT send us any of these risks: Explosives, Trucking of Flammables, Admiralty, Maritime, Jones Act, USL&H, Railroad FELA, delinquent undisputed audit balance, high use of seasonal or day laborors

Program for Some Losses, but in less hazardous classes:
Criteria:
- $2,500 minimum premium
- 2 years continuous WC coverage
- Experience Modification less than 1.39
- If no experience modification calculated, then no more than $5,000 in claims in any one year for premiums under $5,000 and no more than $10,000 in claims in any one year for premiums between $5,000 and $10,000.
- Less than 300% loss ratio in any single year
- Less than 100% loss ratio for the last 2 or 3 years
- Less than 1 claim per 250,000 payroll, all 2 or 3 years combined (rounded down)
- Heights less than 15 feet
- No 24 hour operations/exposure
- Can do code 7228, but no tow trucks
